The Geriatric Psychiatry Acute Dementia unit (2B) team’s approach is grounded in a deep understanding that every patient is a whole person with a history, a culture, an identity and a voice. They provide personalized care that preserves dignity and choice, reduces distress and fosters trust.

The Unit 2B team all take steps to provide the most personalized care possible. Doctors take time to listen deeply to patients and families tailoring treatment plans to honour personal history and preferences. Nurses provide compassionate bedside care, ensuring each patient feels safe, heard and valued. Health care aides build trust through gentle, consistent support with daily activities preserving autonomy wherever possible. Occupational and physical therapists design interventions that respect each patient’s pace, comfort and capacity fostering meaningful engagement.

Creating space to meet patients’ individual needs is regular practice for the team. When a patient with dementia expressed distress at unfamiliar meal options, the nursing team collaborated with dietitians to incorporate traditional cultural foods restoring the patient’s comfort.

The Unit 2B team welcomes families as essential partners in care. Nurses facilitate open compassionate communication ensuring decisions reflect both the patient’s wishes and the families’ insights fostering reassurance and inclusion. The team supports families through the most difficult moments. When a patient passes, staff stand with the loved ones in their grief offering quiet presence and shared humanity.

Among colleagues, the team cultivates a culture where they value every discipline, role and perspective. Interdisciplinary collaboration is seamless because mutual respect is the foundation.
