Lakewood Community Health Centre Operationalization team

The Covenant Community Health Centre is a complex project with many internal and external stakeholders. The operationalization team shared information and updates about milestone completion and hiccups throughout the last two years of the project. They provided exceptional communication and engagement with the program staff teams impacted by the move. Particularly in the final stages of the project, the operationalization team was very intentional in communications and support to clinical staff to minimize impacts to staff, providers, patients and families.

As construction of the building finished, the operationalization team did an immersive current state analysis of all of the clinical areas. They engaged clinical staff and physicians to evaluate what was working well. Additionally, they had process improvement discussions to optimize practices in programs transitioning to the location.

The operationalization team engaged with health services partners and community partners throughout the project. Patients and families were engaged in very early stages of the project vision. The Mill Woods Community Council provided input and feedback. The operationalization team was very intentional about ensuring the community component of this project was not lost and is embedded, not just in the Covenant clinical areas of the building but throughout the space.

The community tile mural at the entrance of the site serves as a powerful symbol of the collaboration and engagement that define this initiative. Each tile represents the contributions of countless individuals whose efforts brought this vision to life.
