Fate Hurtada, registered nurse
Having a passion to see beyond the traditional healthcare model, Fate Hurtada looks instead at a model expressed by the Covenant wellness model. She spearheaded a new initiative called the last wish program. This program offers residents facing end of life and their families an opportunity to engage in a meaningful moment of connection, enhancing their quality of life and bringing joy and peace to their final moments.
Fate works collaboratively with everyone to make a wish happen. She has coordinated wishes such as a helicopter ride, a country music concert, a tattoo, a makeover, a living memorial and more. She explains the program to residents and families and then works with a team to make these wishes come true. In spearheading this program, she increased the level of dignity available to residents and their families at end of life.
When the program was in its infancy, Fate recognized she needed to find a way to give credit to those who assisted in making each last wish a reality. She created a last wish committee and encouraged them to explore ways they could credit those who assisted. The last wish program now provides individual donors, businesses and community groups with a last with certificate that honours their generosity.
Fate also harnesses the power of social media to raise awareness and give credit to those assisting in granting a wish. Her presence on social media also helps demystify hospice and end of life care. This awareness led to a donation of high-end medical beds.
